RAIB have released their report into Surbiton, which highlights issues of corporate memory, sharing and embedding learning all of which are critical factors in preventing incidents. Too often we see organisations spending the majority of their time on the investigation and producing the report without ensuring that the key messages are effectively communicated and learning embedded, to prevent further events.
