This week we had the pleasure of delivering Accident and Incident Investigation training for the Health & Safety team at Morgan Sindall Infrastructure’s Nuclear Business Unit.
The course focused on developing practical investigation skills that go beyond identifying what happened, helping teams understand why it happened and, most importantly, how to prevent it happening again.
Key areas included:
🔍 Understanding the influence of human factors on incidents
🗣️ Improving interviewing techniques to gather high-quality evidence
🤝 Conducting interviews that encourage openness and trust
📊 Analysing evidence to identify underlying and systemic causes
💡 Getting the very best from investigations by turning findings into meaningful organisational learning
Effective investigations are not about assigning blame—they are about uncovering insights that strengthen systems, improve decision-making and create safer workplaces.
Thank you to the Morgan Sindall Infrastructure team for your engagement, openness and commitment to continuous improvement. It was fantastic to see the discussions, challenge and enthusiasm for embedding learning from every incident
